Group Medical Claims
Group Out-Patient Benefits Claim

Documents required to support claims are required to be submitted within 90 days after treatment:

  1.  Completed Group Out-Patient Claim Form
  2.  Original official receipt for each consultation showing the following information:
  • Name of patient
  • Date of consultation
  • Diagnosis
  • Amount charged
  • Attending doctor’s signature with stamp

    3.  Attending doctor’s referral letter (applicable in case of specialist consultation, X-ray and laboratory tests, physiotherapy, chiropractic treatment and prescribed medicine reimbursement)
    4. For Chinese Medicine’s Treatment, original copy of the official receipt and prescription sheet issued  by the Chinese Medicine Practitioner are required.

Group Hospitalization & Surgical Benefits Claim

Documents required to support claims are required to be submitted within 90 days after discharge from hospital:

  1.  Completed Group Hospitalization & Surgical Claim Form.
  2.  Original hospital official receipts.
  3.  A copy of all investigation / laboratory reports issued during the hospital stay, if any.
Group Dental Benefits Claim

Documents required to support claims are required to be submitted within 90 days after treatment:

  1. Completed Group Dental Claim Form
  2. Original official receipts
Group Maternity Benefits Claim

Documents required to support claims are required to be submitted within 90 days after treatment/discharge:

  1.  Completed Group Out-patient Claim Form
    Group Hospitalization & Surgical Claim Form
  2. Original official receipts
Group Life Claims

Documents required to support claims:

  1.  Completed Group Insurance Death Claim Statement
  2.  Original death certificate
  3.  A copy of HK Identity Card or passport of the deceased
  4.  A copy of HK Identity Card or passport of the beneficiary
  5.  Proof of sum insured (e.g., last salary autopay listing)
  6.  Proof of employment (e.g., a copy of employment letter)
  7.  Original Appointment of Beneficiary Form or Letters of Administration or Probate of Will
  8.  Proof of  relationship between deceased and beneficiary
  9.  Address proof of beneficiary
  10.  A copy of medical report or coroner report 
  11.  Sick leave report
  12.  Bond of Indemnity (applicable when the claim amount is equal to or less than HK$ 1,000) 
     
Group Accidental Death & Disablement Claims

Documents required to support claims:

  1. Completed Group Accident Benefit Claim Form
  2. Original death certificate (if applicable)
  3. A copy of HK Identity Card or passport of the insured member
  4. A copy of HK Identity Card or passport of the beneficiary (if applicable)
  5. Proof of the sum insured (e.g., last salary autopay listing)
  6. Proof of employment (e.g., a copy of employment letter)
  7. Original Appointment of Beneficiary Form or Letters of Administration or Probate of Will (if applicable
  8.  Proof of relationship between deceased and beneficiary
  9. Address proof of beneficiary
  10. A copy of medical report, if applicable
  11. Sick leave report, if applicable
  12. A copy of police report, if applicable
  13. A copy of coroner’s report, if applicable
  14. A copy of relevant newspaper article(s), if any
 
Group Total Permanent Disability Claims

Documents required to support claims:

  1. Completed Group Insurance Critical Illness & Total Disability Benefit Claim Form
  2. A copy of medical report
  3. A copy of police report, if any
  4. Sick leave report
  5. A copy of relevant newspaper article(s), if any
     
Group Critical Illness Claim

Documents required to support claims:

  1. Completed Group Insurance Critical Illness & Total Disability Benefit Claim Form
  2. A copy of medical report
  3. A copy of police report, if any
